Symptoms may include itching, pain, or numbness, and erythematous or papular lesions that eventually vesicle and swell. These lesions usually crust over and heal within a few days. Cold sores are generally benign and self-limiting. Antiviral drugs such as acyclovir are only required in patients with primary infection, underlying disease, or an immunocompromised system. Over the years, many treatments have been tried, including those to alleviate symptoms of pain and itching. This also applies to the formulation discussed here. The liquefied phenol and benzocaine preparation exhibits topical anesthetic properties; liquefied phenol has some anti-infective properties, and tannic acid acts as an astringent.

Liquefied phenol is phenol that remains liquid in the presence of 10% water, meaning it contains 90% phenol. Liquefied phenol is a colorless liquid with a characteristic odor; a red color may develop upon exposure to air and light. It burns and whitens the skin and mucous membranes. Its specific gravity is 1.065, and it is miscible with alcohol and glycerin. In the formula discussed here, 0.45 mL may be weighed as 0.48 g if more convenient.

Tannic acid is a yellowish-white to light brown amorphous powder with a strong/sharp astringent taste. It gradually darkens upon exposure to air and light. Approximately 1 g dissolves in about 0.35 mL of water or 1 mL of hot glycerin, and it is freely soluble in alcohol. It is used in concentrations up to 7% in topical gels for cold sores and aphthous ulcers. It is usually applied four times a day.

Benzocaine occurs as small, white, odorless crystals or a white crystalline powder, soluble in water (approximately 1 g in 2500 mL) and in alcohol (1 g in 5 mL). It is a local anesthetic generally applied topically in concentrations between 1% and 20%. It acts only upon contact with skin and mucous surfaces and is available in numerous dosage forms, such as aerosols, creams, solutions, gels, pastes, sprays, and oral lozenges.

Ethanol (alcohol USP, S.V.R.) is a clear, colorless, volatile, free-flowing liquid with a slight but characteristic odor and a burning taste. Its specific gravity is approximately 0.816, and it is miscible with water and many organic solvents. It is primarily used for its solvent properties. Due to evaporation, ethanol provides a cooling effect when applied topically. Owing to its specific gravity of approximately 0.816, 60 g (or 73.5 mL) can be used in this preparation.

Propylene glycol is a clear, colorless, viscous, and practically odorless liquid with a specific gravity of approximately 1.035. It is miscible with water and alcohol and is used as a solvent, preservative, and humectant. In this prescription, 20 g is equal to approximately 19.3 mL by volume. Propylene glycol helps provide moisture to the cold sore area and reduces drying and crusting.

Purified water is one of the many types of water listed in the USP. Purified water is obtained by distillation, ion exchange, reverse osmosis, or other suitable methods. It contains no other added substances.

The specific gravity of substances can be used to easily convert quantities from weight to volume. Since the specific gravity of water is approximately 1, specific gravity is widely used instead of density.

The easiest formula for practical use:

Density=Using specific gravity instead of density:

specific gravity=To convert quantities from mass to volume in this formulation, simply divide the mass by the specific gravity. To convert volume to mass, multiply the volume by the specific gravity. These calculations become especially important in pharmacy compounding practices, particularly in the preparation of parenteral nutrition solutions.

Wishing you a good week…Source: Compounding Articles Book – Pharm.
