Hydroxychloroquine sulfate syrup formulation in antiviral treatment for COVID-19 cases<\/strong>Dear Colleagues;Coronavirus disease<\/strong>An infectious respiratory disease affecting humans caused by severe acute respiratory syndrome coronavirus 2, the disease was first discovered in Wuhan, China in 2019, and since its discovery, it has spread worldwide, leading to the 2019\u201320 coronavirus pandemic.<\/p>\n

Common symptoms observed in the disease include fever, cough, and shortness of breath. Muscle aches, sputum production, and sore throat constitute less common symptoms. Gastrointestinal symptoms such as diarrhea have also been reported.<\/p>\n

Although most cases have mild symptoms, severe pneumonia and multiple organ failure may occur in some patients.<\/p>\n

The virus is usually transmitted from person to person via droplets produced as a result of coughing. Touching contaminated surfaces and then touching one’s own face is also thought to be another method of transmission.<\/p>\n

The incubation period from exposure to the virus until symptoms appear ranges between 2 and 14 days, lasting an average of 5 days.<\/p>\n

Recommended preventive measures include frequent hand washing, maintaining physical distance from other people, and keeping hands away from the face. Mask usage is recommended for individuals suspected of having the virus and those close to them, and was not recommended for the general public.<\/p>\n

With no vaccine and no specific antiviral medication, COVID-19 is managed through symptomatic treatment, isolation, and experimental methods. Research and experimental treatments include vaccine development, the use of existing antiviral medications for COVID-19, passive immunization, and preventing cytokine storm.<\/p>\n

Various antiviral drugs are being researched to treat COVID-19, and some are being used in clinical trials.<\/p>\n

Chloroquine, used in the treatment of malaria, was researched for treatment in China in February 2020, and preliminary results showed the substance to be effective. Hydroxychloroquine inhibits SARS-CoV-2 in vitro. One study stated that chloroquine “improved lung imaging findings, promoted negative tests, and shortened the duration of the disease” in the treatment of COVID-19-associated pneumonia. However, it is thought that the research requires further examination.<\/p>\n

Azithromycin has been investigated as a potential treatment in combination with hydroxychloroquine. A clinical study evaluated 36 subjects confirmed to have SARS-CoV-2 through PCR tests.<\/p>\n

Twenty of the subjects received one or both of the drugs, while the remaining 16 received placebo medications. After six days, virological clearance (testing negative for the virus according to PCR tests) was reported in 100% of cases treated with both azithromycin and hydroxychloroquine, 57.1% of patients receiving hydroxychloroquine alone, and 12.5% of the control group.<\/p>\n

Cytokine storm is a potentially fatal medical complication seen in advanced, severe COVID-19 cases and is associated with excessive cytokine production. There is evidence that hydroxychloroquine exerts an effect against cytokine storms.<\/p>\n

In our country, it is available as Hydroxychloroquine Sulfate 200 Mg film-coated tablets (PLAQUENIL). However, since there is no syrup formulation of this drug, it is possible to prepare a syrup formulation using the tablet.HYDROXYCHLOROQUINE 25 mg\/ml SUSPENSION<\/strong><\/p>\n

Preparation of the formulation:<\/strong>The film coating is removed from the tablet surface using 70% isopropyl alcohol. For this procedure, the tablets are placed in a small dosing cup, and 70% isopropyl alcohol is added just to remove the film coating on the tablet surface. Leave it for 10 seconds. Never leave it for longer than 10 seconds. The tablets are then placed on a paper towel and gently rubbed to remove the film coating on the tablets. The tablets are transferred to a mortar and crushed with a pestle until a fine powder is obtained. The powder mixture is transferred to a conical graduated cylinder. The mortar and pestle are rinsed with a small amount of water to transfer the remaining dose into the graduated cylinder, and simple syrup is slowly added and mixed. Shake thoroughly for at least 30 seconds to ensure a homogeneous distribution. The prepared suspension is placed in an amber bottle. Label properly. Shake well before each use. The prepared suspension contains 25 mg of hydroxychloroquine per ml, and the required dose for the patient should be adjusted and used accordingly. As there are insufficient studies regarding the stability of the suspension, it is recommended to be freshly prepared.<\/p>\n

Hydroxychloroquine sulfate is soluble in water. After the medication is pulverized into powder, simple syrup can be used both to mask the bitter taste and to prepare it as a suspension.Simple Syrup (TFM-01\/2017)<\/strong><\/p>\n

Preparation of the formula:<\/strong>In a tared container, sugar is dissolved in water on a water bath at low heat. Then, stirring continuously, it is brought to a brief boil. The evaporated water is replaced with boiling water. It is strained through cloth or filtered while hot.<\/p>\n

Example: for 200 mL syrup, sugar = 667*200\/1000 = 133.4 g<\/p>\n

133.4 grams of sugar is weighed and dissolved in a warm water bath. The mixture is removed from the heat as soon as it reaches boiling point and left to cool.
