When searching the meaning of the word Lassar on internet search engines, I found it as the Latin equivalent of the word rubber/elastic. I presume this name was given due to the paste-like consistency of this ointment.

To briefly define diaper rash; the medical term for diaper rash is “ammonia dermatitis“. The irritation of the skin with ammonia around the buttocks, groin, and external genitalia is called diaper rash. Although there is plenty of urea in fresh urine, there is no ammonia. Urea does not cause irritation even on the most sensitive skin. However, if the urine stays on the baby’s skin for a while, certain bacteria convert urea into ammonia, and ammonia irritates the skin.

The thin, protective lipid layer on the skin’s surface is penetrated by moisture and waste products (urine-feces), causing the skin to become irritated. To prevent this in the past, mothers used to place leaves, soil, and later cloth under babies that could absorb waste and keep the skin as dry as possible.
Diaper rash is a condition caused by a yeast called Candida albicans. This yeast thrives on the chemistry of feces and urine. The fact that the area is moist and warm also provides a favorable environment for the yeast to proliferate.

Diaper rash usually appears as a slight raised redness at the point where the diaper touches the baby’s skin. When it worsens, red small swellings, blisters filled with fluid, and similar skin changes that cause pain to the baby may be observed. If the diaper rash becomes infected, these skin rashes may take on a bright red color and expand. Small red rashes may spread beyond the diaper contact area.
The most common period of occurrence is in infants aged 9 – 12 months. It starts around the anus, causing erythema on the groin creases, under the penis, the scrotum and its skin, and on the labia majora in girls.

In the affected area, the infant feels stinging, burning, and itching. This leads to constant crying and restlessness.

In infants, a fungal infection may develop on top of the diaper rash. Fungal infections are seen more frequently, especially in infants using antibiotics and after the use of corticosteroid cream. Heat and humidity also increase the risk of fungal infection. When there is a fungal infection in the diaper rash, there are satellite rashes in the form of small, red scatterings around the bright red and moist diaper rash area. In other words, small red spots are seen at the edges of the lesions, and the rash area expands outward.
Unnecessary use of antifungal medications for diaper rash is inappropriate and may even cause side effects. In diaper rash thought to be due to a bacterial infection, the doctor may prescribe an oral antibiotic treatment for the baby.
For allergic reactions, a topical steroid cream or ointment can relieve the rash.

Before moving on to the formulation of LASSAR’s ointment, referred to as PASTE in pharmaceutical technology, let us briefly recall pastes.

PASTE – PASTA’E – PATES

These are thick-consistency ointments containing 50-70 percent solid medication. The powdered substances in them give these preparations mild drying properties. The active substance in such a mass is absorbed more slowly through the skin. Therefore, astringent and antiseptic drugs are very often used in this form. As with ointments, fatty pastes cannot be applied to hairy parts of the skin. While forming a gel in water, they can be applied more easily to wet skin and washed off easily.

The preparation of pastes is just like the preparation of ointments; however, since a large amount of powder is present, the powders must first be thoroughly levigated with a small portion of the melted vehicle or with small amounts of auxiliary substances such as glycerin or liquid paraffin.

LASSAR’S PASTE T.K.

Rp

Zinc (oxyde de)…………… 25.00 g

Amidon de blé ……..,…..25.00 g

Graisse de laine ……..…25.00 g

Vaseline de blanche ..…25.00 g

Analyzing the ingredients in the formula:

Zinc oxide – Zinci oxide – Oxyde de zinc; It is an active ingredient with astringent properties known for its mild antiseptic effects. It is used in eczema, burns, diaper rash, and friction-induced skin damage. Zinc oxide ointment contains 20% and paste contains 25% zinc oxide. Zinc oxide is not absorbed through the skin, and because it does not dissolve in water, it is applied in the form of powder, ointment, or paste.

Starch – Amylum – Tritici amylum – Oryzae amylum; Depending on the source from which it is obtained, starch is referred to as corn, potato, wheat, or rice starch. Starch is used as a binder, diluent, and disintegrant in solid pharmaceutical dosage forms, especially tablets. It is practically insoluble in cold water and cold alcohol. It possesses adsorbent properties.

Lanolin – Graisse de laine – Adeps lanae – Refined wool fat – Lanae cera – Lanoleine T.K.

Obtained from sheep’s wool. It is light yellow in color and has an ointment consistency. Soluble in ether, petroleum ether, chloroform, and boiling absolute alcohol. It can absorb twice its weight in water. It is an emulsifier, forming water/oil emulsions. In its anhydrous state, its consistency is quite firm. With the addition of water, a pasty texture is formed. It absorbs water up to its own weight. It is easily absorbed by the skin and is odorless. It is used as a vehicle.

The vehicle included in the T.K. under the name lanolinum contains 13 parts lanolin, 3 parts liquid paraffin, and 4 parts water.

Vaseline – Vaseline blanche; Known as white petrolatum, it is an emollient ointment base. It is not easily absorbed through the skin. It is obtained by bleaching yellow petrolatum. It is odorless and tasteless.

Preparation of the Formula

The preparation of this formula, included in the European Pharmacopoeia Monographs, is quite simple.

The powdered zinc oxide and starch in the formula (wheat starch is mostly preferred) are mixed until homogeneous and levigated with petrolatum heated slightly above 30 °C until it reaches a white paste consistency. Then, in a separate dish, lanolin heated and softened above 30 °C is added and mixed until a homogeneous preparation is obtained, completing the formula.

Corticosteroid and antifungal ointments can be added to this soothing and protective paste formulation according to need, making it versatile for use in the treatment of diaper rash.

Wishing you a great week…

Specialist Pharmacist Ahmet Nezihi Pekcan
Pekcan Pharmacy – Konya
pekcanecz@gmail.com

Tel: (332) 3520657

http://www.majistralformul.com/

Dear colleagues, this week’s topic is “Lassar’s Ointment”, which is frequently used in the treatment of diaper rash, a condition we often encounter in our pharmacies.
