{"id":9122,"date":"2026-08-11T05:08:32","date_gmt":"2026-08-11T05:08:32","guid":{"rendered":"https:\/\/majistraleczacilari.org.tr\/?post_type=docs&#038;p=2471"},"modified":"2026-08-21T11:51:11","modified_gmt":"2026-08-21T11:51:11","password":"","slug":"local-anesthesia-treatment-with-tetracaine","status":"publish","type":"docs","link":"https:\/\/majistraleczacilari.org.tr\/en\/dokumanlar\/local-anesthetic-analgesic\/local-anesthesia-treatment-with-tetracaine\/","title":{"rendered":"Local Anesthesia Treatment with Tetracaine"},"content":{"rendered":"<h2 class=\"text\"><img fetchpriority=\"high\" decoding=\"async\" class=\"alignnone size-medium wp-image-2472\" src=\"https:\/\/majistraleczacilari.org.tr\/wp-content\/uploads\/2026\/08\/182-300x169.jpg\" alt=\"\" width=\"300\" height=\"169\" srcset=\"https:\/\/majistraleczacilari.org.tr\/wp-content\/uploads\/2026\/08\/182-300x169.jpg 300w, https:\/\/majistraleczacilari.org.tr\/wp-content\/uploads\/2026\/08\/182.jpg 370w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/h2>\n<div><\/div>\n<div class=\"text\">\n<p>Dear Colleagues; this week, we will examine together a preparation from the Spanish National Formulary containing 2% tetracaine for local anesthesia, a topic that has recently come to the fore due to clawbacks jointly imposed by the Social Security Institution on prescribing physicians and compounding pharmacists during retrospective prescription audits on the grounds that the active ingredient tetracaine was used at a concentration exceeding 1% in local anesthetic treatments.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Rp.<\/strong><\/p>\n<p><strong>Tetracaine&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;2%<\/strong><\/p>\n<p><strong>Polyethylene glycol 400&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;70 g<\/strong><\/p>\n<p><strong>Polyethylene glycol 4000&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;30 g<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Substances in the formula;<\/strong><\/p>\n<p><strong>Amethocaine- &nbsp;Pantocaine- Tetracaine- Tetracaine-&nbsp; Tetracainum;<\/strong><\/p>\n<p>A solid waxy, white or slightly yellow substance that is sparingly soluble in water and soluble in alcohol, ether, and chloroform, tetracaine is an ester-type, long-acting local anesthetic. It is considered the most toxic local anesthetic agent compared to other local anesthetics. These preparations are typically used prior to examinations of the larynx, trachea, or esophagus to suppress laryngeal and esophageal reflexes and to provide local analgesia. Additionally,&nbsp;it may also be used for the palliative relief of pain caused by Herpes labialis. Tetracaine, a potent local anesthetic, is used topically in cream and ointment forms. In dentistry, Tetracaine is prepared at 4% and 1.5% concentrations in carbomer gel for the treatment of occlusion. (Occlusion is defined as any contact or the presence of contacts between opposing teeth under the neuromuscular control of the masticatory system.) It must be stored protected from heat and light.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Macrogol 400- Polyethylene glycol 400- Polyoxyethylene 400- Polyethylene glycol 400;<\/strong><\/p>\n<p>Polyethylene glycol 400 is a polyethylene glycol used for various purposes in the pharmaceutical industry. Polyethylene glycols are generally polymers composed of repeating subunits (monomers) of similar structure.&nbsp;Poly(ethylene glycol) is non-toxic and is used in a variety of pharmaceutical products. It forms the base of many laxatives, skin creams (as cetomacrogol), and lubricants used during sexual intercourse (often combined with glycerin). Various types of electrolyte-containing polyethylene glycols are used for bowel preparation prior to surgical and colonoscopy procedures and in cases of drug overdose. Polyethylene glycol 400 (Macrogol 400) is used in the preparation of parenteral, ophthalmic, rectal,&nbsp;topical, and vaginal preparations. Polyethylene glycol 400 is frequently included in the formulation of ophthalmic preparations used for dry eyes.<\/p>\n<p><strong>Macrogol 4000- Polyethylene glycol 4000- Polyethylene glycol 4000- Polyoxyethylene glycol;<\/strong><\/p>\n<p>Polyethylene glycols are water-soluble bases used in topical preparations and suppositories. They are also used as solvents, vehicles, solubility enhancers, tablet binders and lubricants, and as plasticizers in film tablet coating. Certain types, such as Macrogol 3350, are used orally in combination with electrolytes as laxatives. Polyethylene glycols are condensation products of ethylene oxide and water. Polyethylene glycols with an average molecular weight between 200\u2013600 are clear, colorless, or slightly yellow viscous liquids. They have a faint characteristic odor and a bitter taste. Polyethylene glycols with an average molecular weight above 1000 are found as white or off-white solids of varying consistency or as powders. They have a faint characteristic odor. While their viscosity increases with increasing molecular weight, their hygroscopicity decreases. For example, PEG 4000 and higher polyethylene glycols are not hygroscopic.&nbsp;All polyethylene glycols are soluble in water and miscible with each other in all proportions. Liquid types are soluble in acetone, alcohols, benzene, glycerin, and glycols. Solid types are soluble in acetone, dichloromethane, ethanol, and methanol; sparingly soluble in aliphatic hydrocarbons and ether; and practically insoluble in fixed oils and liquid paraffin. Types with a high molecular weight, such as Polyethylene glycol 20,000, form gels in aqueous solutions.<\/p>\n<p><strong>Preparation of the formula;<\/strong><\/p>\n<p>PEG 400 and PEG 4000 are placed in a glass beaker in a water bath and heated until a homogeneous mixture is formed. The heated mixture is added to the tetracaine in a glass mortar using the geometric dilution method and triturated to complete the preparation.
