Dear Colleagues, this week I will share with you a preparation that is available on our pharmacy shelves under the trade name Emla and prescribed for its local anesthetic properties. We can easily compound this formula, which is sold under the same trade name in our country as in various countries around the world, in our pharmacy laboratory conditions.<\/p>\n

We will see together that this preparation, taken from Spanish medical formularies, is a local anesthetic gel with eutectic mixture properties (Eutectic Mixture of Local Anesthetics).<\/p>\n

Substances in the formula<\/strong><\/p>\n

Lidocaine-lignocaine-lidocainum;<\/strong><\/p>\n

Lidocaine is a white to slightly yellowish crystalline powder with a characteristic odor. Its melting point ranges between 66-70\u00b0C. It is practically insoluble in water; very soluble in alcohol, chloroform, or methylene chloride; freely soluble in ether; and soluble in oils. Lidocaine forms a mixture with prilocaine that has a melting point lower than that of either component. This eutectic mixture is used in topical dosage forms. Lidocaine is a widely used antiarrhythmic and local anesthetic. Preparations for local anesthetic use include ointments, gels, sprays, or oral solutions, as well as injectable preparations specific for local anesthesia. The addition of 1:200,000 to 1:100,000 adrenaline to lidocaine solution slows the vascular absorption of the drug and prolongs its duration of action. When applied topically to mucous membranes, its effect starts within 1-2 minutes and lasts for about 30-60 minutes.<\/p>\n

Prilocaine hydrochloride-citanest hydrochloride;<\/strong><\/p>\n

Prilocaine hydrochloride is a white, odorless or almost odorless crystalline powder. Its melting point is 166-169\u00b0C. According to the British Pharmacopoeia (BP): Freely soluble in water and alcohol; practically insoluble in ether. According to the United States Pharmacopeia (USP): Soluble 1:3.5 in water, 1:4.2 in alcohol, 1:175 in chloroform; very slightly soluble in acetone; practically insoluble in ether. According to the USP, the pH of its injectable solution is 6.0-7.0. The melting point of the mixture formed by prilocaine with lignocaine is lower than the individual melting points of both substances (eutectic mixture). This eutectic mixture is used in topical dosage forms. Prilocaine hydrochloride is an amide-type local anesthetic mainly used in dentistry. With an intermediate duration of action, prilocaine has a longer duration of action compared to lidocaine. It produces less vasodilation compared to an equal dose of lidocaine.<\/p>\n

Preparation of the Formula<\/strong><\/p>\n

First, the gel that will carry the local anesthetic substances is prepared. 1% CARBOPOL 940 is left to swell in distilled water. While forming the gel, antimicrobial preservation is provided with 0.08 g of nipagin and 0.02 g of nipasol. In addition, a few drops of triethanolamine are added to the resulting gel to neutralize it.<\/p>\n

The formula is completed by adding the mixture of lidocaine and prilocaine base, which we mixed in equal amounts and is called a eutectic mixture, into the prepared gel.<\/p>\n

Directions for Use<\/strong>
\nThis preparation is obtained by mixing two active substances in equal amounts. As a result of this eutectic mixture, a gel containing 5% local anesthetic substance is produced. This gel has a fairly wide range of uses.<\/p>\n

It is used where topical skin anesthesia is required, particularly for needle punctures and superficial surgical procedures in hemodialysis patients; for superficial anesthesia before superficial surgical procedures and debridement in leg ulcers (for example, cleaning parts with fibrin, pus, and necrosis); for superficial anesthesia of the genital mucosa; in post-herpetic neuralgia; for local anesthesia before epilation in women; and in premature ejaculation in men.<\/p>\n

The preparation should be stored in a light-resistant glass container. 1 ml of emla should be applied in a thick layer per cm of the skin to be treated; if necessary, covering the application area with an adhesive bandage and waiting for half an hour before the operation or surgical procedure is important to increase the expected local anesthetic effect. The area where the gel is applied must be cleaned and disinfected before the surgical procedure.<\/p>\n
