Left untreated, these microscopic mites can live on your skin for months. They reproduce on the skin surface, then burrow into it and lay eggs. This causes an itchy, red rash on the skin. Scabies, which is transmitted from person to person through contact predominantly in crowded environments such as hospitals, nursing homes, military barracks, and care facilities, is not transmitted to humans from animals like cats and dogs, contrary to popular belief. Spreading more easily especially among individuals who wear the same clothes, share the same bed, or share the same towel, scabies is transmitted regardless of age and gender. After exposure to scabies, it may take up to six weeks for symptoms to appear. Symptoms usually develop much faster in individuals who have had scabies before. The hallmark symptoms of scabies are a rash and intense itching that worsens at night. Continuously scratching the affected area due to itching can break the skin and lead to sores that become infected.

Common areas where scabies is seen in older children and adults are:

– Wrist
– Elbow
– Armpit
– Breast
– Penis
– Waist
– Buttocks
– Area between fingers

Areas affected by scabies in infants or young children, and sometimes in the elderly or immunocompromised individuals, are:

– Head
– Face
– Neck
– Hands
– Soles of the feet

The rash itself may consist of small bites, burrows, lumps under the skin, or pimple-like bumps. Parts of the mites’ burrows may sometimes be visible on the skin. They may appear as small raised or discolored lines.

Dear Colleagues; Benzyl benzoate is an acaricidal drug used in the treatment of scabies. We will examine this formula from the Spanish National Formulary together.

Rp.

Benzyl Benzoate 200 g

Stearic acid 20 g

Triethanolamine 6 g

Distilled water csp. 1,000 g

Substances included in the formula:

Benzile benzoato [FU]- Benzylbenzoat [ABDATA-SK]- Benzylis benzoas [IP3]:

Benzyl benzoate occurs as colorless crystals or a clear, colorless, and oily liquid with a faintly aromatic odor. It is practically insoluble in water and glycerol; miscible with alcohol, chloroform, dichloromethane, ether, fixed and volatile oils. It is applied externally in concentrations of 10% to 30%.

Application is performed after bathing by applying the medicine all over the body below the neck; a second application is made after 5 days if necessary. Before using the lotion, the whole body is washed with a mild soap and warm water. The lotion is applied onto warm skin. It is left to dry without rinsing. It is not applied to eyes and mucous membranes. In this treatment, careful disinfection of personal clothing and bedding must be performed.

Alternatively, three applications are made at 12-hour intervals. To prevent re-infestation, all laundry must be changed with clean items. Its use is generally not recommended in children; in necessary cases, more dilute solutions may be used to reduce the risk of irritation. Benzyl benzoate is also used as a pediculicide. It is additionally used as a solubilizing agent.

Stearic acid- Acide stearique- Octadecanoic acid:

Stearic acid is used as a lubricant in tablets and capsules, and as an emulsifier and solubilizing agent in topical preparations.

Stearic acid is defined in the British Pharmacopoeia (BP 1993) and the United States National Formulary (USPNF XVII) as a mixture of stearic and palmitic acids. The stearic acid content in this mixture is at least 40% and the total amount of the two acids is at least 90%. Stearic acid is a white or light yellow, slightly glossy, crystalline solid, or a white or yellowish-white powder. It has a slight odor and a taste resembling tallow. It is practically insoluble in water; soluble in ethanol, hexane, and propylene glycol; freely soluble in benzene, carbon tetrachloride, chloroform, and ether.

Triethanolamine- Trihydroxytriethylamine- Trolamine:

Triethanolamine is an excipient used for various purposes in different types of pharmaceutical formulations. It is mostly used in topical pharmaceutical formulations in emulsion form. When mixed in equimolar proportions with fatty acids such as stearic acid or oleic acid, non-ionic soaps with emulsifying properties are formed. Thus, visually appealing and stable emulsions with a pH of approximately 8 are obtained. It is used in injectable preparations to prepare salt forms of active pharmaceutical ingredients. In some formulations, it is also included as a buffer, solvent, polymer stabilizer, and humectant. Although generally regarded as a non-toxic substance, it may cause irritation and hypersensitivity on the skin and mucous membranes. It is less irritant than monoethanolamine and ethanolamine.

Triethanolamine is actually an alkanolamine mixture. It consists of high amounts of triethanolamine and low amounts of diethanolamine and monoethanolamine. It is a clear, colorless, or pale yellow, viscous, hygroscopic liquid. It has a slight ammoniacal odor. It is miscible with water and alcohol; soluble in chloroform; slightly soluble in ether. Its 10% aqueous solution gives an alkaline reaction to litmus. It should be stored in airtight containers and protected from light.

Triethanolamine reacts with mineral acids to form crystalline salts and esters. It forms water-soluble salts with high molecular weight fatty acids. These salts have the general characteristics of soaps. It reacts with copper to form complex salts. In the presence of heavy metal salts in its solutions, discoloration and precipitation occur. It reacts with halogenated reagents such as thionyl chloride, and very toxic products are formed as a result of the substitution of the hydroxyl groups in its structure with halogens.

Preparation of the formula:

In a glass beaker, stearic acid is dissolved in benzyl benzoate on a hot water bath at a temperature of 70 C. In another beaker, triethanolamine is added to distilled water and heated at the same temperature. Then, the aqueous phase is poured onto the oily phase and stirred rapidly with a glass rod.

Wishing you a great week…

Source:

1-La Formulación Magistral en la Oficina de Farmacia, 1.a parte

2-RxMediaPharma®2018 Interactive Drug Information Source

Specialist Pharmacist Ahmet Nezihi Pekcan
Pekcan Pharmacy – Konya
pekcanecz@gmail.com
Tel: (332) 3520657
http://www.majistralformul.com/

Benzyl benzoate in the treatment of scabies It is a skin disease caused by a mite known as Sarcoptes scabiei.
