Today, when we look at pharmacy storefronts, it is very common to see signs such as “Dermocosmetics in particular, Cosmetics, Aromatherapy, Mother and Baby Products, Medical Supplies, Herbal Drugs, Sports Supplements” and the like. However, there is another storefront sign we are looking for and wish to see: “Compounding Formulas”<\/p>\n

We might think, ‘Compounding formulas is not a privilege; is there any pharmacy that does not prepare compounded prescriptions, so why should we feature this sign on our storefronts?’…<\/p>\n

This is where the issue begins… In order to genuinely say that I prepare MAGISTRAL FORMULAS, I would like to briefly mention the standards we must possess, namely the ideal VALIDATION CRITERIA (measurement procedure) that should be present in pharmacy medication compounding.<\/p>\n

To briefly summarize these criteria:<\/p>\n

1- Rather than being showcase spaces that we only remember from inspection to inspection, not exceeding 3-4 square meters and consisting merely of a faucet and a sink, our laboratories should have sterile standard equipment of at least 5-6 square meters, equipped with modern devices such as a hot water bath, heating plate, distilled water apparatus, electronic balance, distillation apparatus, powder sieves, capsule filling machine, label printer, etc., used solely for their intended purpose; and periodic calibration measurements of the devices used in these laboratories must be performed.<\/p>\n

2- The raw materials we use in compounding medications in our laboratories must have passed quality control and possess a standard of quality suitable for pharmacy use. In this regard, raw materials approved by a SCIENTIFIC BOARD must be used. It should not be possible for raw material importers or manufacturers to supply raw materials that have not received SCIENTIFIC BOARD approval to pharmacies. A method should be developed regarding raw material procurement, and if possible, distribution should be carried out through a single source. Thus, we will achieve standard raw material quality in the medications we prepare in our pharmacies. Differences in raw material quality varying from pharmacy to pharmacy will be eliminated.<\/p>\n

3- Formulas in the prescriptions arriving at our pharmacies vary, and writing and dosing errors are occasionally encountered. We, community pharmacists, must now move away from the role of merely being a pharmacist who dispenses the prescription brought to the pharmacy. In other words, we must transition from being passive to taking an active role, remembering our identity as a MEDICATION EXPERT as required by our academic education, and contribute to the development of these formulas. By submitting technical and scientific experience-based contributions regarding formulas from our pharmacies to an academic committee to be established by the SCIENTIFIC BOARD, just as in Germany (NRF – German New Prescriptions Formulary) and the Netherlands (FNA – Dutch Pharmacists Formulary), we must also establish and develop the TURKISH PHARMACISTS FORMULARY<\/strong> in our country. With this developed formulary book, instead of formulas whose preparation varies from pharmacy to pharmacy, a standard and continuous reference book will be available both on the desk of the prescribing physician and in the hands of the compounding pharmacist, thereby improving the quality of TREATMENT WITH MAGISTRAL FORMULAS and expanding formula diversity.<\/p>\n

4- Finally, to mention what the preparation process of a compounded formula in a pharmacy should entail: just as in developed countries, magistral formulas should be compounded by a pharmacist according to an available “ready manufacturing protocol,” continue with the completion of this protocol, and conclude with a “control protocol.” I have full confidence that these operations\u2014through the manufacturing and control protocol process, appropriate physical conditions, and laboratory equipment and devices\u2014will enhance the prestige and well-deserved respect of the pharmacist in the healthcare sector by utilizing their power rooted in production, and will initiate a new era in terms of the seamless compounding in pharmacies of medications that occasionally face manufacturing and distribution challenges for various reasons.<\/p>\n

Yes, esteemed colleagues, we spoke of a new era: I have tried to summarize from a different perspective how the preparation of MAGISTRAL FORMULAS, which is seen in our pharmacies as a traditional practice or a pharmacist’s hobby, should truly be carried out. In this process, I have tried to outline the duties falling upon our professional organization, the Turkish Pharmacists’ Association, our academic colleagues at universities, and of course, us community pharmacists.<\/p>\n

With these thoughts in mind, I would like to thank Ms. \u00d6zlem Demir, editor of the Eczac\u0131n\u0131n Sesi e-newspaper, for providing the opportunity to share my website named ‘Majistral Form\u00fcl’ with you, my esteemed colleagues,<\/p>\n

and I extend my highest respects to my professor, Prof. Dr. Tamer Baykara, who contributed to the formation of this concept with his valuable writings.<\/p>\n
