The formula is also included in the 1977 British Pharmacopoeia and the British Veterinary Pharmacopoeia.<strong>Rp.<\/strong><\/p>\n<p><strong>Cetrimide&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;5 g or sufficient quantity<\/strong><\/p>\n<p><strong>Cetostearyl alcohol&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;50 g<\/strong><\/p>\n<p><strong>Liquid paraffin&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;500 g<\/strong><\/p>\n<p><strong>Purified water, freshly boiled and cooled&nbsp;&nbsp;&nbsp;&nbsp; to 1000 g<\/strong><\/p>\n<p><strong>Substances included in the formula:<\/strong><\/p>\n<p><strong>Cetrimide [INN; Ph. Eur.; ASK]- Cetrimidum [Ph. Eur. 4; Ph. Int. III]- Cetrimide:<\/strong>It is a white or almost white, voluminous, free-flowing, combustible powder with a characteristic odor. It is freely soluble in water, alcohol, and chloroform; practically insoluble in ether. A 2.0% w\/v solution in carbon dioxide-free water foams strongly when shaken. It is incompatible with soaps and other anionic surfactants, bentonite, iodine, phenylmercuric nitrate, and alkali hydroxides. Its aqueous solutions react with metals. According to the British Pharmacopoeia (BP), Strong Cetrimide Solution (Strong Cetrimide Solution BP 1993) contains 20-40% cetrimide and up to 10% alcohol, isopropyl alcohol, or both as a preservative. For use, a 1% aqueous solution is prepared from this concentrated solution. This prepared solution can be sterilized in an autoclave. Sterilized preparations should be used within 7 days after opening and discarded if this period is exceeded. Cetrimide is a quaternary ammonium antiseptic. It exerts a bactericidal effect against Gram-positive and (at higher concentrations) Gram-negative bacteria. However, particularly some Pseudomonas species and Mycobacterium tuberculosis are resistant to quaternary ammonium compounds, including cetrimide. Cetrimide is ineffective against bacterial spores. Its antifungal activity varies, and it may be effective against some viruses. Like other quaternary ammonium compounds, especially benzalkonium chloride, it is used for washing and cleansing the skin, wounds, and burns. For this purpose, 0.1-1% solutions or 0.5% creams are used. Its solutions are usually obtained by diluting more concentrated stock solutions. Cetrimide is often used in combination with chlorhexidine rather than alone. This combination in lotion form is beneficial in the treatment of acne. Cetrimide and benzalkonium chloride are also included in the formulation of eye drops and solutions used for cleaning hard contact lenses; however, they should not be used for the same purpose with soft contact lenses. In addition, hair shampoos containing 1-3% cetrimide are used against dandruff in cases of seborrheic dermatitis. Cetrimide is also included in the formulation of some emulsifying preparations (Cetrimide Emulsifying Ointment BP 1993).<strong>Cetostearyl alcohol- Cetostearyl alcohol [BP; NF; P.Cx.79]- Lanette(R) O:<\/strong>Cetostearyl alcohol occurs as a white or pale yellow waxy mass, or as white flakes or granules with a faint characteristic odor. When heated, it melts into a clear or pale yellow liquid free from turbidity or dispersed particles. It is practically insoluble in water; freely soluble in ether; soluble in alcohol and petroleum ether. When melted, it is miscible with fixed oils, liquid paraffin, and melted lanolin. Cetostearyl alcohol is a mixture of solid aliphatic alcohols consisting predominantly of cetyl alcohol and stearyl alcohol. It is obtained by the reduction of the appropriate fatty acids. The British Pharmacopoeia (BP) and the United States National Formulary (USNF) state that it must contain not less than 90% cetyl+stearyl alcohol and not less than 40% stearyl alcohol. Cetostearyl alcohol is used as a stiffening agent and emulsion stabilizer in creams, ointments, and other topical preparations. When combined with hydrophilic emulsifying agents, it is useful in preparing oil-in-water type emulsions that remain stable over a wide pH range. It is also used to enhance the emollient effect of ointments formulated with paraffin. Although it is essentially a non-irritant substance, hypersensitivity reactions associated with its main components, cetyl and stearyl alcohol, have been reported.<strong>Liquid paraffin- Liquid petrolatum- Liquid vaseline- Mineral oil [INCI; NF; NF16; USP]- Paraffinum liquidum [INCI; EU]:<\/strong>Liquid paraffin is widely used in topical pharmaceutical preparations. It is included in the composition of ointment bases to benefit from its emollient properties. It is used in oil\/water type emulsions. It is used as a lubricant in tablets and capsules. Due to its lubricating properties, it is also used in ophthalmic formulations. Liquid paraffin is a mixture of saturated liquid hydrocarbons obtained from petroleum. It is a transparent, colorless, odorless, and viscous liquid. It does not fluoresce in daylight. It is a tasteless and odorless substance, but emits a faint odor when heated. It is practically insoluble in water, glycerin, and alcohol; soluble in benzene, acetone, chloroform, ether, carbon disulfide, and petroleum ether. It is miscible with most volatile and fixed oils (except castor oil). The addition of small amounts of surfactants enhances miscibility\/solubility. It should be stored in airtight containers and protected from light.<strong>Purified water, freshly boiled and cooled:<\/strong>Distilled water is highly purified water prepared by the distillation of drinking water and used as a solvent or vehicle in many pharmaceutical formulations. It is a clear and colorless liquid with no characteristic odor or taste. In pharmacy, water refers to distilled water. Distilled water has a limited shelf life. In any case, it must be boiled before use. Since additional specific properties are required for water to be used in parenteral preparations, water for injection should be used for this purpose. In addition, deionized water can also be used instead of distilled water in permitted pharmaceutical preparations, as it is cheaper and easier to obtain. In this formulation, the process of boiling and cooling the water is intended to eliminate dissolved oxygen from the water to prevent the formula from oxidizing.<strong>Preparation of the formula:<\/strong>Melt the cetostearyl alcohol in a low-temperature water bath and add liquid paraffin brought to the same temperature. Meanwhile, dissolve the cetrimide in freshly boiled and cooled distilled water and add it to the oil phase at an equal temperature. Remove the mixture from the water bath and stir until it reaches room temperature to prepare the formula. A phosphate buffer can be added to this composition. Cetrimide can be used in proportions of 0.5%, 1%, and 2%.
