Burow’s Cream in Subacute Dermatitis

Dear Colleagues,  This formulation containing Burow’s solution, also known as Aluminum Acetate solution, is preferred in subacute dermatitis and  pruritic skin diseases. Preparations containing aluminum acetate are known as Burow’s creams, emulsions, lotions, or solutions. If we examine this formula quoted from the Australian Pharmacopoeia;

 

Aluminium acetate cream oily, Burow’s cream

Aluminium acetate solution .. 5 ml

Zinc oxide ……………………..20 g

Wool fat ……………………….25 g

Arachis oil …………………….25 ml

Purified water, freshly boiled and cooled….27 ml

Substances in the formula

Burow’s solution – Aluminium acetate – Aluminum acetate topical solution: Aluminium acetate solutions are astringents. In dermatology, aluminium acetate solutions are also included in the composition of astringent lotions prepared to reduce skin irritation.

Zinci oxidum – Zincum oxydatum – Zinc oxide – Oxyde de zinc: Zinc oxide is a white or yellowish-white, odorless, amorphous, soft, particle-free powder. It slowly absorbs carbon dioxide from the air. It is practically insoluble in water; soluble in dilute mineral acids. Zinc oxide is a mild astringent with weak antiseptic properties. It is applied topically in cases of eczema, abrasions (scrapes, chafing), burns, friction-induced skin damage, diaper rash, and mild skin irritations. Since it does not dissolve in water, it is applied in the form of powder, ointment, or paste. Zinc oxide ointment contains 20% and zinc oxide paste contains 25% zinc oxide. Zinc oxide is also used in combination with coal tar or ichthammol in the treatment of eczema.

 

 

Wool fat – Anhydrous lanolin – Purified lanolin – Adeps lanae anhydricus – Refined wool fat: Lanolin is a water-free, pure waxy substance obtained from the wool of sheep (Ovis aries, Bovidae). It is included in the composition of special moisturizing and skin-softening creams. It is an important excipient used in pharmaceutical technology. It is used in the preparation of water-in-oil type creams and ointments. When mixed with suitable vegetable oils or petrolatum, softening creams that penetrate the skin are obtained. Due to this property, it facilitates the absorption of drugs. Lanolin mixes with approximately twice its weight of water without phase separation, forming stable emulsions. 10 g of lanolin must hold at least 20 ml of water. It is practically insoluble in water; slightly soluble in cold alcohol, very soluble in boiling alcohol. It is freely soluble in benzene, chloroform, ether, and petroleum ether.

Oleum arachis – Peanut oil – Huile d’Arachide – Oleum arachis: According to the European Pharmacopoeia (Ph. Eur. 7), refined peanut oil (Arachis Oil, Refined; Arachis Oil BP 2012) is obtained from shelled peanut seeds. It is a clear, yellowish viscous liquid composed mainly of glycerides of oleic and linoleic acids and a small amount of other acids. It solidifies at approximately 2 °C. It is very slightly soluble in alcohol; miscible with petroleum ether.  It should be stored in well-filled containers. It must be protected from light.

Purified water, freshly boiled and cooled: Boiled and cooled purified water.

Preparation of the formula

Zinc oxide is triturated in a glass mortar with peanut oil until it becomes soft and smooth. Lanolin, slightly melted in a hot water bath, is added to this mixture and stirred until homogeneous. After mixing the aluminium acetate solution with the calculated amount of previously boiled and cooled purified water, it is gradually added to the mixture in the mortar forming the oil phase, completing the preparation.
