Burn Ointment / Unguentum Contra Combustiones

Dear Colleagues, in this week’s article, we will examine together a highly effective formula prescribed in Germany for first- and second-degree burns.

Pantocaini                                  0.1 g

Extracti Hamamelidis fluidi     5.0 g

Liquoris Aluminii acetici         20.0 g

Eucerini anhydrici              ad  50.0 g

Substances in the formula;

Amethocaine –  Pantocaine – Tetracaine – Tetracaine –  Tetracainum;

Tetracaine, a solid waxy, white or slightly yellowish substance that is poorly soluble in water and soluble in alcohol, ether, and chloroform, is an ester-type, long-acting local anesthetic. It is considered the most toxic local anesthetic drug compared to other local anesthetics. These preparations are typically used for local analgesia and to prevent laryngeal and esophageal reflexes prior to examinations of the larynx, trachea, or esophagus. In addition, it can also be used for the palliative relief of pain caused by Herpes labialis. Tetracaine, being a potent local anesthetic, is used topically in cream and ointment forms. In dentistry, Tetracaine is prepared in carbomer gel at concentrations of 4% and 1.5% for occlusion therapy. (Occlusion is defined as the presence of any contact or contacts between opposing teeth resulting from the neuromuscular control of the masticatory system.) It should be stored protected from heat and light.

Extrait fluide d’hamamélis – Witch hazel (Hamamelis virginiana L.) leaf – Hamamelis leaf – Hamamelis virginiana e foliis;

Witch hazel leaf (Hamamelidis folium) consists of the dried leaves of witch hazel (Hamamelis virginiana L.), belonging to the witch hazel family (Hamamelidaceae), containing at least 7% tannins. It is prepared by percolation extraction of the used plant part with a water-alcohol mixture. The herbal drug is defined in the European Pharmacopoeia. Hamamelidis folium contains tannins, gallic acid (a bitter substance), and a very small amount of essential oil. As its main chemical constituents, it contains catechins such as (+)-catechin, (+)-gallocatechin, (-)-epigallocatechin gallate, as well as cyanidin- and delphinidin-type proanthocyanidins. Witch hazel leaf (Hamamelidis folium) exhibits astringent and hemostatic effects. It possesses local vasoconstrictive properties. Its ointments and suppositories are used in the treatment of hemorrhoids. Due to its astringent and cooling effect, its aqueous distillate is included in the formulation of creams used against skin burning and itching. It promotes healing in minor skin wounds and relieves soreness in dry and cracked skin. It is used for diaper rash in infants and young children, minor burns, and sunburns. Hamamelis virginiana aqueous distillate has also been used for its hemostatic effect. Hamamelidis Folium has also been used internally in the symptomatic treatment of varicose veins, such as varices and hemorrhoids.

Witch hazel leaf has no known contraindications. Its use requires no special warnings or precautions. No interactions with other drugs have been reported.

Burow’s solution – (Aluminum acetate topical solution);

It is a clear, colorless liquid with a faint sour odor, a sweetish, astringent taste, and a pH of 3.6–4.4. It contains approximately 5% acetic acid and is generally used as an astringent dressing to treat conditions such as fungal infections, dermatitis, diaper rash, dry skin, otitis externa, and poison ivy dermatitis. Dilutions of the solution between 1:10 and 1:40, as well as its semisolid preparations, are widely used as wet dressings.

Lanolin ointment – Eucerin anhydricum – Unguentum Alcoholum lanae – Wollwachsalkoholsalbe;

Lanolin ointment, according to the German Pharmacopoeia (DAB);

Cetostearyl alcohol    0.5 g

White petrolatum        93.5 g

Lanolin                  6.0 g

Preparation of the formula;

Pantocaine is dissolved in Burow’s solution. Witch hazel extract is added to the prepared solution. Then, the solution is added into the ointment base prepared by gentle heating, and stirred until a homogeneous mixture is obtained.
