Aluminum Subacetate Solution in the Treatment of Eczema / DermatitisDear Colleagues; Formulas containing aluminum subacetate are among the compounded preparations we most frequently prepare in our pharmacies.

Characterized by a light yellow color and a faint acetic acid odor, this liquid is used as an antiseptic and astringent in the treatment of exudative eczema.

Colloquiallyreferred to as ‘mayasıl’among the public,eczemais a skin disease that develops suddenly and is characterized by tissue lesions such as redness, itching, weeping, crusting, etc.It is defined as an acute or chronic inflammatory condition typically characterized by redness (erythema), formation of fluid-filled blisters (papule and vesicle formation), scaling, thickening, and hardening (lichenification) on the skin, appearing in successive stages in most cases. It may sometimes be accompanied by hyperpigmentation, itching, and burning. Commonly known as ‘atopic eczema/dermatitis’, this clinical condition is not a single disease, but a combination of several diseases with shared characteristics. A more appropriate term describing this is atopic eczema/dermatitis syndrome (AEDS).Formula;

Aluminum subacetate 5%        50 g

Substance in the formula;

Aluminum diacetate – Basic aluminum acetate – Aluminum subacetate – Sous-acetate d’aluminium:Aluminum subacetate (basic aluminum acetate) is an astringent substance used topically. It is generally included in the composition of ointments used for hemorrhoids. This solution, which has an antiperspirant (sweat-inhibiting) effect, is also used as a drying and deodorizing agent. Aluminum salts should not be used in cases of severe renal failure due to the risk of aluminum toxicity. Aluminum is absorbed orally and can accumulate in the body. Preparations containing aluminum salts should not be used in patients with renal disease or failure unless signs and symptoms of toxicity and serum levels are closely monitored. In addition, it should be considered that citrates present in the composition of many effervescent preparations will increase aluminum absorption from aluminum salts.

According to T.K. (1954), SOLUTIO ALUMINII SUBACETATIS must contain at least 8.5% aluminum subacetate.

Aluminum sulfate 100 parts

Calcium carbonate 46 parts

Dilute acetic acid 120 parts (contains 29.7–30.6% acetic acid)

Water q.s.
After the preparation is prepared, assaying should be performed, and the amount of aluminum subacetate contained in the preparation along with the control date must be recorded on the label.

It is stored in the pharmacy as an officinal preparation (as a stock solution). With the aid of this stock solution, more dilute aluminum subacetate solutions can be prepared. In this case, calculations are performed as in the examples shown in the formula above, and the preparation is adjusted to the desired concentration.Preparation of the formula:The solution readily available as an 8.5% stock solution should be calculated at a rate of 5% in this formula and made up to the desired weight with distilled water.Calculation:Amount of active ingredient in 50 grams of a preparation containing 5% active ingredient:

5×50/100 = 2.5 grams.

If an officinal preparation at a concentration of 8.5% (aluminum subacetate sol.) is used in preparing the formulation, the amount to be taken from the stock solution for the 2.5 grams of aluminum subacetate required for the prescription is,

2.5×100/8.5= 29.41 grams.Method of Preparation:Weigh 29.41 grams of the 8.5% officinal preparation into a tared container and make up to 50 grams with water.

Note: A cloudy solution may form due to the precipitation of basic salts as a result of high temperatures in the storage environment.
