IN INFLAMMATORY SKIN DISEASES (in the treatment of subacute pruritic eczema)
5% – 10% COAL TAR AND ETHANOL-CONTAINING ZINC OXIDE LOTIONDear Colleagues,
In line with the high demand from you this week, especiallyECZEMA for the treatment of which, I thought of sharing a formula that is standardly prescribed in Germany.
As you may recall, in my article dated 07/04/2014,Ethanolic Lotion with Ichthammol and Zinc Oxide in Inflammatory Skin Diseaseswe examined a formula together under this title. Unlike that formula, coal tar replaces ichthammol in this one, while the vehicle base of the lotion remains mostly the same.Rp.
Pix lithanthracis …………………….……5.0 g
Zinkoxid …………………….………… 20.0 g
Talkum ………………………………. 20.0 g
Glycerol 85 % ………………..……….20.0 g
Ethanol 90 % (v/v) ….………………20.0 g
Greinigtes Wasser …………..ad..100.0 g
If we examine the ingredients included in the formula;
Coal tar-Goudron de Houille-Coal tar-Pix Carbonis-Pix Lithanthracis- Steinkohlenteer; It is prepared by the dry distillation of coal. It has a naphthalene-like odor and a burning taste. It is a dense, near-black liquid. It solidifies upon contact with air. It is slightly soluble in water. While it dissolves in chloroform and benzene, it is partially soluble in alcohol and ether. Its density gradually increases through the action of air. It burns with a luminous, sooty flame. Its saturated solution exhibits an alkaline reaction to litmus. Coal tar is included in the formulation of medicinal shampoos, soaps, and ointments used for dandruff and psoriasis, as well as for killing and repelling head lice. Soluble extracts (2%) prepared by dry distillation of coal tar are also used in the treatment of dandruff. Tars and tar oils reduce the thickness of the epidermis. They are antipruritic and exhibit a mild antiseptic effect. They are used topically in the treatment of eczema, psoriasis, dandruff, seborrheic dermatitis, and other skin diseases. Coal tar preparations have largely been replaced by wood tars. Ultraviolet light enhances the efficacy of coal tar in the treatment of psoriasis. Some wood tars are included in the composition of expectorant preparations. Tar has also been used in homeopathic medicine.Zinc oxide-zinci oxide-oxyde de zinc; It is an active ingredient with astringent properties, known for its mild antiseptic effects. It is used in eczema, burns, diaper rash, and friction-induced skin damage. Zinc oxide ointment contains 20% zinc oxide, while zinc oxide paste contains 25%. Zinc oxide is not absorbed through the skin, and since it is insoluble in water, it is applied in the form of powder, ointment, or paste.
Talc-Talcum-Purified talc; Talc is a natural hydrated magnesium silicate. Its approximate chemical formula is “Mg6Si2O54OH4. It may contain varying amounts of aluminum silicate and iron. It is a very fine, light, homogeneous, white or off-white or grayish-white, odorless crystalline powder. When in contact with the skin, it readily adheres to the contact surface. Purified talc must be free from microscopic asbestos particles. Talc is practically insoluble in water, mineral acids, and dilute solutions of alkali hydroxides.Glycerol-Glycerine-Glycerol; – 85%; Glycerin is a clear, colorless or very pale, hygroscopic, syrupy, odorless or almost odorless liquid with a sweet taste. It is miscible with water and alcohol; slightly soluble in acetone; practically insoluble in chloroform, ether, fixed oils, and essential oils. Its aqueous solutions are neutral to litmus. According to the United States Pharmacopeia (USP), the pH of Glycerin Oral Solution, USP is between 5.5 and 7.5. Strong oxidizing agents form explosive mixtures with glycerin.Purified water; Distilled water
Preparation of the formulaCoal tar is placed on a paper coated with liquid paraffin, weighed on a watch glass, dissolved by stirring in a portion of the distilled water specified in the formula, and 90% ethanol is added. Zinc oxide and talc are mixed in a mortar and softened by gradually adding glycerin. The ethanolic coal tar is added to the mixture in the mortar, and the formulation is completed with the addition of the calculated amount of distilled water. Care should be taken to ensure the total mass of the formulation is 100 g.
In this formula, coal tar proportions can be prepared between 5% and 10% as specified in the formula.Application and storage conditions; It is applied to the affected area 2-3 times a day in subacute pruritic eczema. Treatment can be continued for up to 4 weeks. The preparation is quite stable. It can be stored for up to 3 years in well-closed glass or plastic containers.
Wishing you a good week…
