Dear colleagues;
Before writing about this formula, known as SOLUTIO CASTELLANI and mostly prescribed by ENT specialists, I would not want to pass without commemorating the Italian scientist ALDO CASTELLANI, who gave his name to the preparation.
Born in Florence on September 8, 1877, the scientist Aldo Castellani discovered the reaction known as “Absorption”. He investigated the causes of sleeping sickness in Uganda and established the Ceylon bacteriology laboratory. He identified the disease “hemorrhagic bronchopirochetosis” and the causative agent of framboesia (yaws). He directed first the New Orleans Institute of Tropical Pathology and then the Clinic for Tropical Diseases in Rome. On October 3, 1971, he passed away in Lisbon at the age of 94. Castellani’s solution, which is used in fungal skin infections and named after him, is our topic of this week.
If we examine CASTELLANI’S SOLUTION according to German medical formulas (DRF);

c) Acidi borici pulv………………………….. 1.0 g

Preparation; should be evaluated in two parts.
SECTION A: The preparation begins by adding acetone to 10 g of 10% alcoholic fuchsin solution. Dissolution should be carried out by stirring occasionally and waiting for at least 2 hours.
SECTION B: On the other hand, boric acid is dissolved by heating in a portion of distilled water. Resorcinol is added to this solution and the solution is left to cool. Liquid phenol is added to the solution.
Then, the solution prepared in SECTION B is added to the solution prepared in SECTION A and completed to 100 grams with the remaining distilled water. The preparation must be filtered.
Since the topically applied solution has an intense burgundy-red color, it may leave a dye stain around the ear where it is applied that does not come off for days.
The solution should be prepared as fresh as possible and used fresh. Therefore, instead of obtaining it from warehouses that sell compounded preparations, it should be purchased as raw materials and freshly prepared properly in a laboratory environment.

If we examine the substances used in the formula;
Fuchsini: It is a dye, used as an antiseptic and antifungal.
Phenol: Effective against mycobacteria and certain fungi. It also has local anesthetic and analgesic effects.
Acidi borique: Has bacteriostatic and fungistatic effects.
Aceton: Used as a solvent in the treatment of skin diseases.
Rezorsin: Has antiseptic, disinfectant, and antifungal effects.
Treatment; CASTELLANI’S PAINT contains acidifying and dehydrating agents. External auditory canal mycosis – otomycosis is a mycotic infection characterized by loose, cotton-like, thin tissue that is easily detached, with colors ranging from white-yellow to green-black. Fungi are either primary pathogens or superinfected on bacterial infections. Otomycosis is the result of prolonged bacterial otitis externa that alters the flora of the external auditory canal. Therefore, they are frequently mixed infections characterized by S. aureus, Pseudomonas, and Proteus species. Fungi rarely act as primary pathogens, especially in the presence of excessive humidity or heat. Infections are often asymptomatic. Usually, there are complaints of itching and a feeling of fullness in the ear. The itching is severe enough to cause excoriations on the epidermis.
With the patient in a lying position, 5-10 drops are instilled into the ear two to three times a day, and after a while, cotton should be placed over the ear and the patient should sit upright. Since the preparation contains a dye, the possibility of staining linens should not be overlooked. Treatment should be continued for at least 7 days.
Although CASTELLANI’S PAINT is known, as I mentioned at the beginning of my article, as a preferred formula prescribed by ENT specialists in the treatment of otomycosis, it is also frequently used in the treatment of the following diseases:
It is a formulation frequently used in PYODERMA (IMPETIGO CONTAGIOSA), FOLLICULITIS, FURUNCLE, ECZEMA, SEBORRHEIC ECZEMA, THRUSH, and FUNGAL SKIN DISEASES.
The preparation should be stored in tightly closed dark brown glass bottles away from heat and light.
